Do you need to install any runtime apps to use Maxi-Patch_AV. From what I remember I think had to install Max/MSP runtime to use Maxi-Patch_3.6.7.

No need. Maxi-Patch_AO/AV are self-contained. If on Windows, however, you need to install QuickTime for Windows (apple.com) and some dorky support library from Microsoft:
