Each instance of Pinky VST requires a stereo pair of inputs, and a stereo pair of outputs. The easiest way to set it up in Ableton is just to allocate one stereo audio channel per instance of the plugin. The following three images show the basics:
Be sure to enable Monitor "In" for each audio channel where an instance of the plugin lives.
